Insert a column to the far left, name it "{label}" (the first column
will be the label with Google Spreadsheets, whether convenient or not)
and have its contents be the concatenation of the provider and product
columns. You can likely achieve that automatically using spreadsheet
formulas, looking something like (assuming cell A:2) =concat(B:2,C:2).
Alternatively, insert a column elsewhere, name it {id} and set up its
contents the same way.
