I'm not sure if I understand your problem. When I look at
http://simile.mit.edu/exhibit/examples/nobelists/nobelists.html
in Firebug, I only see one <head> and one <body>.
As for __history__.html, you just need to create such a file in the same
directory as the html file. It's a dummy file used in a trick to track
the browser's back and forward navigations.
