I also wanted to add to this that RefSeq is a curated set and each record
will most likely have a HUGO gene symbol unless there is none assigned to
the locus.

The geneName field in the refFlat table comes directly from the NCBI record
for that ID.  The contents of the geneName field are the result of whatever
the submitter inserted into the record, so it may not necessarily be the
hgnc gene symbol.
