Seems fairly straightforward: gtk+ requires cairo to be compiled with 
the X and pdf USE flags set.

echo "x11-libs/cairo X pdf" >> /etc/portage/package.use ; emerge 
--oneshot x11-libs/cairo x11-libs/gtk+
