Thanks for you work and the great report.I think the PAX error is a known issue. At least there were known issues resulting in detected refcount overflows.
I did not check the changelog, but using a later grsec/pax pacthset might help.
As a sidenote: I'd prefer going with LTS kernel 3.14 as this is declared LTS by both GKH for the kernel and spender/pipacs for grsec/pax patchset.
