My bet is that you are also running qmail-scanner correct?  I have had
this happen with qmail-scanner and basically what appeared to have been
the culprit is when qmail-scanner tried to inject the mail for
delivery... something failed.  qmail-inject received the email but
qmail-scanner didn't realize it so it tagged for delivery again...

Sorry to be vague with my interpretation, but it was quite a while ago.
