I believe this is due to a permissions problem. Make sure everything in
the qmailscan directory is owned by the user under which you are running
qmail-scanner (on my system it's qscand). I think that is set in a
config file in /etc/conf.d but I can't remember for sure. If you do a ls
-l on /var/spool/qmailscan at least something in there will probably
already be owned by the user you want.

PS: note that there should be a qmail-scanner.log in the
/var/spool/qmailscan directory as well.
