Have you checked that somehow you dont have your start up scripts duplicated in both your /etc/runlevels/boot and /etc/runlevels/default? You could check for files with the same names. If they exist, move (am avoiding delete) the ones from /etc/runlevels/default that are also in /etc/runlevels/boot.
