I did this yesterday. It was painful.
I went from a two days old up-to-date ~x86 nptl to a x86 system. things
that break :

gcc couldnt compile gcc, something broke mid emerge -u system.
installing binary gcc x86 fixed, and let gcc rebuild itself.

once glibc was updated, mount didn't work and broke, forced a rebuild
there before things worked.

openssl : after downgrading openssl, rebuild gcc.  Gcc will cache
sanitized headers of openssl that broke when updating wget. ergo, wget
was killed.

Tips:

before you start installing, download all things.
