You need to edit the e-build. If you want to install "timidity++-2.12.0" the file you would edit is "/usr/portage/media-sound/timidity++/timidity++-2.12.0.ebuild". If you look at the file you will notice a number of config options are already being enabled, check if any of them are the ones that you want and you could also remove any of the config options you don't need.
