Easiest thing to do is install dboxfe (dosbox frontend) and configure the settings there. What is your MIDI setup (is MIDI currently running on your machine without reference to dosbox)? I should have ALSA MIDI and Timidity set up (I think), but I haven't tested as I don't offhand know what DOS games/apps actually use MIDI. So if you happen to know of a piece of abandonware that does, let me know and I'll try it, and if it works, tell you how I did it (if simply setting DOSBox up correctly doesn't work for you).
