Well, I was hoping someone that knows more about udev would post something. This is what I would check into. Did you upgrade udev recently? If so, try downgrading and rebooting. If that doesn't help, I would delete the udev rules and reboot. They should be in /etc/udev/rules.d/.

If neither of those helps, maybe try a newer version that is not supposed to be stable. I am using the same version of udev you are but I don't use lvm so I can't say that it works, just that it works for me without lvm.

If you are still stuck, post again with what you tried. Maybe someone will come up with something.
