This is not a Nominatim bug. El Salvador is tagged with is_in=North
America. See: http://www.openstreetmap.org/browse/node/424311802.
In any case, there is no agreement whether the Americas are one or two
continents. When considered as two continents El Salvador is generally
counted as part of North America. So please don't change this until you
have talked to the OSM community and have gotten some kind of agreement.
