It looks like there is no county named 'Kottayam' in OSM. If it is not
mapped, nominatim cannot use it. Please note further that the county of
Alappuzha is currently mapped as a place node. As long as the counties are
not mapped with their boundaries, nominatim may get the county still
wrong.
