For the upload form:
template (includes javascript):
http://github.com/GeoNode/geonode/blob/master/src/GeoNodePy/geonode/templates/maps/layer_upload.html
handler:
https://github.com/GeoNode/geonode/blob/master/src/GeoNodePy/geonode/maps/views.py#L879

We're currently rewriting the upload form on synth, so if you intend to pull
in changes from there you should put the check into the handler.

For the metadata form:
python view:
http://github.com/GeoNode/geonode/blob/master/src/GeoNodePy/geonode/maps/views.py#L665
template:
http://github.com/GeoNode/geonode/blob/master/src/GeoNodePy/geonode/templates/maps/layer_describe.html

Hope that helps.
