The first one means that there is an entry in the metadatabase for at
tile, but the tile does not exist on disk. This could be due a number
reasons, for example that it has been deleted, or that the server
suddenly shut down. It's not unusual to have a few of these.
The second one means that your client is making requests that do not
match the grid for which tiles are made. Have a look at the demo clients
on http://localhost:8080/geoserver/gwc/demo , if you use OpenLayers you
can copy that code.
