GeoServer can render layers perfectly well using an SLD with four rules.
 Usually when a layer is blank in the preview it's a problem with the style
(maybe you have an error in your Filter elements keeping any features from
matching any of the rules?)

The approach I usually take to troubleshooting issues goes like this -

1. Load layer using appropriate default style (go to preview, add
'&styles=polygon' to the URL).  Does it render?  If no, double check
connection parameters, bounding box, geometry type, etc. in layer settings.

2. Edit problematic style: comment out all but one rule, and comment out the
minscaledenominator, maxscaledenominator and filter in that.  Re-add these
elements one by one, reloading the preview at each step.  At some point, all
the features for the rule you're editing will disappear, you just found the
problem.
