I think adding the string directly in the SLD may suit you (
http://docs.geoserver.org/stable/en/user/styling/sld-reference/labeling.html#composing-labels-from-multiple-attributes).
Another
option may be using the Parametric Substitution in SLD functionality (
http://docs.geoserver.org/stable/en/user/styling/sld-extensions/substitution.html?highlight=parametric
).
