All the big deploys run on top of Oracle or PostGIS as far as I know.
I'm talking about clusters of GS serving hundreds of millions of rows.

The SQLServer store is maintained in spare time but there is, as far as I know,
no developer actually using it in production.
PostGIS is maintained because all devs like it, Oracle is maintained because it
drives business, SQLServer seems to be stuck in a limbo where no core developer
is actually using it, no one using it wants to step up and become a maintainer,
nor it drives any kind of business (that I know of).

I believe we'll move forward the day someone decides to put the time/money where
the mouth is and get it pushed up to the PostGIS/Oracle level.
