Hi, the problem is not image size (though 200MB PNG will kill you in other ways, see the rest of the mail), but the fact "force declared crs" does not work for rasters: http://jira.codehaus.org/browse/GEOS-4583 I'm cooking a patch for it as I write, but regardless, you can work around the issue by adding to the file set the attached projection file (and then remove the layer, clear the resource cache in the "server status" page, and configure the layer back). That should make the layer appear properly. That said, a 200MB PNG will have disastrous performance, PNG is not a good format to feed to any map server. Please use gdal_translate to translate it to a TIFF with inner tiling (eventually using JPEG compression if you're short on space) and then use gdaladdo to add various overviews so that GS does not have to read all the data just to display a timestamp sized map.
