Phil,
I had a similar phenonomenon, i.e. offset at some scales in a different
setup. Reason was that the geowebcache.xml and an Oplenayers function call
didn't have the same bounding box.

maxExtent:new
OpenLayers.Bounds(1786000,1997264.499195665,3953471.00160867,3081000)};
The numbers in OpenLayers.Bounds were the offending bit.
