I also get a 404 if I browse to tile 18/9/-1
The tile calculations are off.
It seems as you might mix different SRS in maxExtent and resolutions.
The resolutions seems to match those of EPSG:900913 and maxExtent looks
like EPSG:4236 ones.
What SRS do you state in your spec to mapfish?
