This happens on JPEG as well as PNG, and at random locations.  If I reseed it dissapears from original location but again appear in new location.  Totally random.  We run Geoserver 2.11.0 on Ubuntu 16.04 with imageio native extension installed.

I don't remember seeing this particular problem, the block shape and distribution is... perplexing :-)
So no clue, I'm going to make some more questions to help reduce the context of the issue.
Does it happen in all caching formats? png vs jpeg in particular
Do you have more installation details, like for example, OS, if you have imageio native extension installed?
