Hi, That spatialite community module has been made with Spatialite version 2.4.0 and current development of Spatialite happen in version 4.4.0. I would say that forget that outdated extension but if you absolutely want to use it then it might work better with a Geoserver version that is from the same historic time.
Geoserver 2.12 has now support for GeoPackage in core.
