Hi Fawzy,
For my part I found it simpler to just deploy MapFish separately. This
was a few years ago; I've not been as active with GeoServer so may have
missed it, but I don't believe the plugin is anywhere near current, so
I'd definitely suggest a standalone MFP deployment.
To answer your questions for MFP 2:
1) http://www.mapfish.org/doc/print/configuration.html#page-definition
"The “header” and “footer” sections are optional. If the “items” that
are in the main section are too big, more pages are generated. The
header and footer will be drawn on those pages as well."
2) Just include "PNG" in the formats:
"?formats:
- pdf
- png"
I've not used MFP 3, so would simply point you to the docs -
