They can be used both, but the specific examples you cite won't work
together:

   - A rendering transformation replaces the source data the style is used
   against, you call the SLD on top of a set of points, and, say, "heatmap"
   turns it into a raster (so your style must be for the raster)
   - A geometry transformation takes a single geometry from a feature and
   applies a function to get some other geometry (e.g., from line to centroid
   of the line), if there is a rendering transformation it will be applied on
   the results of the transformation

Long story short, they should work together, as long as the RT generates
vector data, and the geometry transformation is meant to work on top of the
results of the RT (not its inputs, it will never see them):

   - pointstacker -> centroid(line) works but the centroid is applied on
   the points extracted by the point stacker, doing nothing (the centroid of a
   point, is a point)
   - heatmap -> centroid(line) does not work, because centroid cannot be
   applied on a raster dataset
