It seems recently someone implemented a user space R-Tree for H2:
http://hatbox.sourceforge.net/
Which would basically work the same way that SpatiaLite R-Tree indexing works.
Did some initial experimentation and initial results look good. Bounding box
queries of the texas roads dataset come back pretty fast.
