Just noticed from the stack trace that you are using WFS;

The database will be PostGIS then; pgadmin is just the admin program. It may
make a difference depending on what constrains are in place for the table; were
you able to verify that the request makes it through WFS-T protocol and over to
the server? Have a look in the server logs for a better strack trace.

I doubt you are doing much wrong; it is more a case of carefully checking each
leg of the journey and making sure the data is valid at each step along the
way.
