This is usually caused by missing or incomplete coordinate reference system
(i.e. CRS for short) information. Or in rare cases using a system that we have

Ah - that would be the incomplete part.
What you can do is:
a) look up the official CRS that "matches" the one you have, the official CRS
has additional parameters including these Bursa wolf parameters
b) you can sometime ask it to proceed by allowing an in-exact transform
