What operating system are you using?  What version of Gerbv are you 
running?

I am on Windows 8.1.  I just downloaded a recent build of Gerbv from 
SourceForge.  It's gerbv 2.6A, built on July 13, 2016.

Both PDF and SVG work for me, (and have worked for at least 3 years) 
however, I'm not at all pleased with the results.  I won't enumerate all 
of the issues.

If you are on Linux or Mac, hopefully the following advice will give you 
some idea of what the issues are if you want to print full scale (and at 
full resolution) on a printer.

The first thing that I needed to know was the resolution of my printer. 
I found the information on a spec sheet, 600 DPI.  Perhaps your printer 
is nice enough to show the resolution when you print a test page.

Instead of printing the PCB image from Gerbv, I export it to PNG.  The 
PNG output format is capable of reproducing exactly what you see on the 
screen.  It's important to set the desired resolution (600 dpi in my 
case) down at bottom of the PNG output dialog.  The default resolution, 
0, results in a really poor rendition of the PCB.

Next I use IrfanView, which is a Swiss Army knife for images. 
(Unfortunately, IrfanView is only available on Windows, though with some 
fiddling, it has been known to run under Wine on Linux.)  On the Image 
menu in IrfanView, I select Information.  I fill in the Resolution 
fields and click on Change.  (PNG files include metadata that most 
programs, not just Gerbv, fail to fill in.)  Depending on what utility 
you use to print your PCB image, you may or may not have to do this 
step, but it's what works for IrfanView.

Finally, I print from IrfanView.  I select "Original size (from image 
DPI)" for the "Print size".  If you use a different utility, you may 
have to do something like specify an image size, based on your board 
dimensions.

On Windows anyways, most other utilities that can print PNG files have a 
mind of their own, and they won't do what you tell them to do, even if 
they have the options to do so.  IrfanView is the only program that I 
have found that doesn't misbehave.
