[general] patch subject naming convention

There are a lot of [PATCH] subjects out there and it is hard to
look at them all.

Can we adopt a convention of

 [PATCH][subject] Blah BLah Blah

so we can scope these a bit more. eg.

 [PATCH][jsr77] Fix the bug.
