The biggest problem I have found with open source projects is when you send fixes/changes to the list and they disappear into a black hole. You never know whether it is because the code is poor, someone just hasn't dealt with it, or because you have a funny name :-)
The bug reporting tool at Jira (see the Wiki/FAQ for the link) is used for this purpose. The idea is that that way, patches can never get lost (though they may also not be applied :-)
At least it solves the idea of submitted patches disappearing, which is one of the things that you mentioned.
