I'd be concerned that any separate list will instantly begin to diverge from the true picture.

Would it not be better (if we choose to do this at all) to come up with a scheme for attaching such metadata to the file itself either somehow in the repository or by a mandatory comment in it's head etc...

I like the mandatory comment - it would be easy to police, all changes to such data would be versioned, it could be javadoc-ed, etc...

Is this something that was suggested by a retained legal counsel or is
it an idea that came up in a discussion.  I have say that this is a
burdensome request and it is not clear in my mind that it's required or
even useful to fend off legal inquiries.

If it's a mechanism to puts everyone's mind at ease, I think that there
has got to be a better way as this seems to be a highly unorthodox
request.
