It also happen to me. I just rename my app file in the Finder (say I add a space at the beginning), I validate the new name, the icon appears and I remove the space I added. Now the icon is permanent.
I think it is depending on the version (I don't recall if it's about Mac OS or RB or both). I'm using RB 5.5.5 on 10.4.8.
