Review Comment:
For `INGEST_MODE_APPEND` we indicate that if the parameter is not specified,
there is an implicit precedence to target the temp table if both temp and
non-temp tables exist with the provided name. None of the other cases have
special semantics for unspecified vs `false`, so I'm just stating here
explicitly that they should be treated the same in those cases.
