Review Comment:
   @abtom87 Most of the follow-up comments look addressed now. I think this may 
be the one remaining gap: `safe_accumulate_word()` returns `false` for negative 
`word_len`, but `concat_ws_impl()` only checks `state.overflow`, so it looks 
like a negative valid input length could still slip through.
   
   Would you mind checking how is that case handled, please?
