Thanks for the careful review against the cache design, @akshaychitneni! I put all four fixes into this PR in 81ff2e31f rather than a follow-up (replies inline). On tests, there are new scheduler tests for (1), (3) and (4), plus the client tests you suggested. One switches a single session between blocking and pipelined with `SET` and checks the results match.
