Rebased onto master (1308c624c2) to resolve the conflicts with #13928.
Range-diff against the previous head, 58c923178d:

   | Previous | Now | Change |
   |---|---|---|
   | 86ce559e22 traffic_ctl: grade config reload results by level | ae6da44ac3
| Same `src/` and `doc/` changes; only the tests moved, see below. |
   | 85e6bdef72 Fix the order get_all() documents | c4cd32f9f3 | Unchanged. |
   | 36bd707580 Narrow the TrafficCtlStatus.h include | 0a4ef56aa7 | Unchanged.
|
   | 58c923178d Note the task bound on a malformed reload level | b4aefe0572 |
Unchanged. |

Test changes in the first commit:
   - `config_reload_helpers.py`: master's version, with `return_code` added to
`require_command` and `wait_for_status` on top of its root-task check.
   - `test_virtualhost_remap.py`: master's version, unchanged by this PR. It
now runs `config reload --monitor` on the failed reload and expects 2, which
this PR keeps.
   - `test_traffic_ctl_reload_error_level.py`: `test_reload_error_level` allows
the two `cache.config` errors its reloads cause, now that the Uranium ATS fails
a test on an `ERROR:` line it does not expect. The timeout test spells out
`--initial-wait`, since `-w` is the global `--watch` after #13861.

The description is updated to match.
