When trying to find errors, check https://commons.wikimedia.org/w/index.php?title=Special%3ALog&type=gwtoolset&user=Ndalyrose to see if anything is listed for your recent uploads. It seems like a lot of them failed due to a file already existing with the name you chose.
&'s are allowed in titles/urls. However in an XML file they have to be encoded (e.g. written as &).
