I think I've let this sit for long enough now.
Discussion was short, with only one response: Ross Paterson suggested that
the name be `mapAccumRWithKey` instead of just `mapAccumR` as the latter
is available through `Traversable` with a different type.
I agree with this change and attached is the revised patch.
