I see this kind of mysterious behaviour on Windows a lot. Recently I
thought I'd tracked it down to the realtime virus scanner, when I found
that turning off the virus scanner made the problem go away. However I've
now seen this happen even in directories that are excluded from virus
scanning.
It's as if the OS sometimes has a slightly delayed view of the filesystem
(a file that is deleted is still there, or a file that has been created is
not).
See also #2924, which we put down to the virus scanner, and #3231 which is
still not diagnosed.
