Ticket #4834, which proposes to make `Applicative` a superclass of
`Monad`, contains a patchbundle for ghc
(attachment:ghc_new_monad_hierarchy.dpatch:ticket:4834) that adds
`Applicative` instances for all monads in ghc. The patchbundle also
includes some auxiliary refactoring patches.

asked] if I could create a separate ticket for this patchbundle. So here
it is.
It would be great if someone could review these patches.
