I can get reimbursed up to $100 for new glasses, but the form that needs to be completed asks for provider's tax ID #, License # and signature. Any one had good luck getting one of these forms completed by any of the internet companies?
I've called my insurance provider, BC/BS, and they aren't set up to deal with internet providers.
