The first line is your right eye ("oculus dexter"). The second line
is your left eye ("oculus sinister"). The third line is the amount of
power in your bifocals or progressives.
Your right eye has a cylindrical correction (for astigmatism); 15
degrees is the axis of the cylinder. Your left eye has a spherical
correction for mild myopia (nearsightedness).
To use Zenni Optical's form as an example, the columns are laid out
so:
OD-SPH OD-CYL OD-AXIS OD-ADD
OS-SPH OS-CYL OS-AXIS OS-ADD
So your entries, if your numbers are correct as you wrote them, would
be:
0 -0.75 15 +2.00
-0.75 0 0 +2.00
You have to be absolutely precise in how you enter your prescription,
taking care to check that the plus and minus signs are where they need
to be along with the right numerical values.
