I recommend you check out the ClearLight Lenses at $39.

When it comes to shorter lens heights I prefer free form lenses. The
only places I know that offer these are $39 and CoastalContacts. Only
$39 offers relensing.

I recently ordered a new pair of progressives from Coastal with a lens
height of 28mm and used their free form lenses. The mid range is way
more comforatable than a pair I own with Varilux Ellipse. Incidently
the Varilux came from $39 so this is another option for shorter lens
heights.

Free form lenses such as the Varilux Physio 360 will cost you a small
fortune. I have the ClearLight in a pair of frames that measure in at
32mm. As I said before, the mid range is extremely comfortable, and
reading and distance vision is excellent.

I don't think your cousin's prescription should pose a problem. The
ClearLight lenses have an index of around 1.58 which should help him.
These lenses can be tinted.
