in the right eye you have a spherical correction of +1.25 and a
cylindrical correction (for astigmatism) of +1.50 on an axis of 4
degrees with a near vision add of +2.50 (for bifocals or progressives)
wih a prism of 1.00 base down

in the left eye you have a spherical correction of +1.25 and a
cylindrical correction (for astigmatism) of +1.25 on an axis of 180
degrees with a near vision add of +2.50 (for bifocals or progressives)
wih a prism of 1.00 base up

you would enter for the right eye:
Sphere +1.25 Cylinder +1.50 Axis 4 Prism 1 UP  Add +2.50

you would enter for the left eye:
Sphere +1.25 Cylinder +1.25 Axis 180 Prism 1 DOWN  Add +2.50

This is a pretty standard way to write a prescription, btw, the x 4
and x 180 are the degrees on which the cylinder correction for
astigmatism lies in your prescription and this would be found in any
prescription where there is astigmatism correction.

Be aware that many of the online retailers do NOT make glasses with
prism correction so you will have to shop around.  I know that
opticals4less does.
