I get some emails that are HTML that show the paper-clip icon but there is
no attached file.  Something in the HTML code must be triggering the icon
to appear.

You can always examine the full code of the email message ("Show original"
in drop-down menu next to the Reply icon in upper right).  If you know your
HTML, you can see if there really should be an attached file, and you could
probably extract that portion of the code and run it through a decoder to
retrieve the missing attachments.  I occasionally had to do that, back in
the darker ages of the Internet (~1990), but haven't had to with Gmail.
