If the intended recipient has their emails automatically forward to a
second address (which is becoming a very common thing these days), and that
second address is inaccessible, it can cause that to happen.

Alternatively, if another address was put in the BCC field, and that
address bounces, it can seem as if it was sent to one address but bounced
from another.
