[GMT-401] Added comments to bring the code to spec, fixed an initialization
problem that was producing crashes due to a sporadic buffer overrun (this is
bug GMT-401), reworked the initialization to be cleaner and to handle the
isInitialized flag correctly, and generally scrubbed the code.
