 I added some more stuff, in particular, video display card
make/model, and connection type.

 I'm putting the time at 10 AM to 3 PM.  I'm ass-uming things will go
faster the second time around, since we've worked out some of the
bugs.  Thoughts?

 I discovered at work today that MSIE 6 wasn't interpreting my CSS as
intended; I have to figure out why and fix that.

 I don't think so.  That part of the form has been re-vamped in any
event, to make the questions (hopefully) clearer.

good question, particularly in the four states that we list in our box.

 I Googled for "cable providers" and added a few that I saw that
might service this area, but I'm sure that's not a complete list (for
example, Metrocast wasn't there).

And we will have to have lots of information supplied, but I am not sure
that we want it on this page.

 Absolutely.  Forms should be mostly for data entry.  I've added a
few "I've RTFM" questions, and put the date and time on there (mainly
so any potential future events are distinguished), but that's about as
far as I plan to go with the form itself.

 One of the great things about web forms is hypertext can be used for
explanation.  I've put in some dummy hyperlinks, pending actual
instructions.

I would suggest a page that is separate from the "MythTV  page" ...

 Same thoughts here.  It may even make sense to have more than one,
but we can figure that out as we make it up.

On the "Installathon page" we are very strict with the information:

 Just to be clear, I'm not volunteering to do anything more
sophisticated than an HTML form and a mail-based or flat-text-file-log
for submissions.  Someone else will have to do the SQL database with
real-time 3D visualization charts and all that.  ;-)
